Because navigating the in’s and out’s of cancer treatment can be difficult as this is a time of uncertainty and adjustments. By definition, “survivorship” begins from the date of diagnosis and continues throughout the continuum of care. The Cancer Nutrition Care team created the Optimum Nutrition Programs with our client’s unique questions and concerns in mind. Throughout this in-treatment program, clients will work directly with a Dietitian who is a Board Certified Specialist in Oncology Nutrition. The focus will be on creating a plan of action and periodic guidance to promote survivorship, while providing counseling and coaching for any treatment-related side effects that might occur. Your program begins with a 60-minute initial assessment & planning session and includes three additional 30-minute follow-ups.

How does this work and what to expect?

You start by picking a date and time for the initial 60-minute assessment and planning session. You will have a choice to do this by phone call or secure video session. Your Oncology Dietitian will then send you an intake form via email to complete in preparation for your session. In advance, please feel free to complete any of our additional free screenings listed on our website (e.g. Critical Nutrition Screening, Quality of Diet Screening, and Supplement Review). Your assessment, individualized plan of care and any additional supporting documents will be shared with you in detail during the first session and may be accessed through your secure client portal.

Because the transition following cancer treatment can be a time of uncertainty and adjustments. “Survivorship” begins from the date of diagnosis and continues over time as one strives to find their “new normal”. The Cancer Nutrition Care team created Optimum Nutrition Programs with our clients in mind. Throughout this post-treatment program, clients will work directly with a Dietitian who is a Board Certified Specialist in Oncology. The focus will be on providing a plan of action and periodic guidance to promote survivorship through the adoption of a more “cancer protective diet”. Counseling and coaching on any treatment-related side effects that are still lingering is included as well. Your program begins with a 60-minute Initial Assessment & Plan and includes three additional 30-minute follow up sessions.
